Crime-scene investigator blamed for evidence errors had prior problems but returned to work

The Houston crime-scene investigator blamed for errors that could jeopardize dozens of criminal cases was investigated once before because of suspected mistakes, including shoddy work on two officer-involved shootings, according to an internal review released Thursday.

The results, compiled in a May 2016 report, led to additional forensics training and clearance for the investigator to return to work at the Houston Forensic Science Center, the independent crime lab that processes evidence for hundreds of cases each year.

The investigator’s mistakes came to light this week in a crime lab audit sent to the Harris County District Attorney’s Office that outlined issues with key evidence in 65 cases, including 26 homicides, five officer-involved shootings and six child deaths since 2015.

Dozens of Errors by Houston Crime-Scene Investigator – NBC 5 Dallas

A review of crime-scene work shows a Houston police investigator made errors in 65 cases, including homicides and officer-involved shootings, dating back to October 2015.

The Harris County district attorney’s office said in a statement Wednesday that it’s notifying defense attorneys of the errors.

Authorities say the scope of the problem was revealed in a recent audit of the work conducted by the unidentified officer.

The audit covered 88 cases and found numerous instances of incomplete documentation, administrative mistakes and misplaced evidence.

The cases include 26 homicides and five officer-involved shootings.

Authorities say the officer has been reassigned to a patrol division and a supervisor who did not notice various errors has been temporarily removed from oversight duties.

A Houston police officer working in the Crime Scene Unit is under fire after making 65 careless errors while investigating a host of crimes, including murders and officer-involved shootings.

The Houston Forensic Science Center announced Tuesday morning that it had booted the officer from crime scene investigations and he has been transferred to a patrol position. Thanks to his oversights, the Harris County District Attorney’s Office is now notifying the defense lawyers in every case affected, just in case the paperwork omissions or inconsistencies and the misplacement of some physical evidence might affect the outcomes of the cases. The Harris County District Attorney’s Office would not speculate about just how badly the unidentified officer’s mistakes will affect the 65 cases.

“Defense lawyers are going to have a chance to determine for themselves the relevance of these errors or omissions,“ Tom Berg, first
